Patient’s Guide
To Prepare for Admission
  1. To be referred by the attending physician.
  2. Please bring the following for registration at Admission Office, G/F of Block A:
    •Admission Documents / Budget Estimate Form / Insurance Claim Form (if any)
    •Identity Document
    •Relatives or guarantor’s ID card or passport (If patient is under the age of 18)
    •Deposit, medical insurance card or a letter of guarantee from the approved credit account or insurance company
  3. Please bring along your personal amenities, pajamas, slippers etc. Supplies by the Hospital are chargeable items.
